Health care and social service providers treating or assisting HIV+ individuals are encouraged to refer them to Illinois HIV Care Connect. By enrolling in Illinois HIV Care Connect at no charge, people living with HIV receive help in finding the care and support they need. How people living with HIV enroll confidentially<\/h3>

Your referral will put the HIV+ individual in contact with your Illinois HIV Care Connect regional office to enroll confidentially. On the state map on this webpage, just click on your region to find the office\u2019s regional project director, telephone number, and address.<\/strong> In most cases, the person living with HIV brings insurance and income information to an appointment with an office staff member, who helps to submit the enrollment information.<\/span>

After enrollment<\/h3>

Once enrolled, a case manager will determine the individual\u2019s eligibility for services and benefits from programs including the\u00a0AIDS Drug Assistance Program (ADAP Medication Assistance)<\/a>, Ryan White Part B, Continuation of Health Insurance of Coverage Program (CHIC Premium Assistance)<\/a>, Housing Opportunities for Persons Living With AIDS (HOPWA)<\/a>, and others. In this way, Illinois HIV Care Connect can help individuals better afford the health care and support services they need to manage HIV and achieve optimal health and self-sufficiency. In many cases, services will be available near an individual\u2019s home.<\/span>
